A 41 YEAR OLD MALE WITH ABDOMINAL DISTENSION AND SHORTNESS OF BLOOD

I have been given this case to solve in an attempt to understand the topic of "patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with diagnosis and treatment plan.
A 41 year old male patient of suryapet came to OPD with chief complaints of abdominal distension and SOB on eating or working ,speech distortion ,joint pains while walking on 20/10/2021.
CHIEF COMPLAINTS
Patient complains of abdominal distension since 7 days
shortness of breath since 7 days
unable to walk properly since 7 days.
HISTORY OF PRESENTING ILLNESS
Patient was asymptomatic until 1 week ago then developed abdominal distension with no pain and have speech disorientation and SOB.
constipation and reduced urinary output was seen.
PAST HISTORY
No similar complaints in the past
Not a know case of hypertension, diabetes.
PERSONAL HISTORY
Adequate diet before developing of symptoms
mixed diet
sleep no adequate
no consumption of alcohol and smoking
FAMILY HISTORY
No history is seen in family
No cases of hypertension and diabetes
TREATMENT HISTORY
Previously seen by a doctor and on prescribed medication no improvement is seen.

Patient is cooperative and conscious.
